Do you need to apply for Social Security disability benefits?
If you have not yet applied for Social Security disability benefits, you should contact your local Social Security field office. Visit the Social Security Office Locator to locate the office nearest you.
You can also apply for Social Security Disability benefits online. You must complete both an application and an Adult Disability and Work History Report. These forms are available at the Social Security Administration website.
If you wish to apply for Supplemental Security Income (SSI), you can complete the Adult Disability and Work History report online, but you must complete the SSI application in person or over the phone. To contact Social Security by phone, call 1-800-772-1213.
